Health Records on iPhone: A Guide for MEDITECH Patient Portal Members

Door County Medical Center (DCMC) today offers Health Records on iPhone to patients who are Meditech portal users, bringing together hospitals, clinics and the existing Apple Health app to make it easy for patients to see their available medical data from multiple providers whenever they choose.

Health Records on iPhone now available to consumers enrolled in MEDITECH’s patient portal

Visit myHealthDoylestown to view your lab results, update personal information, request an appointment and more. This patient portal is a secure, convenient tool, available 24/7, that enables you to be proactive and informed about your health.

You can be granted access to another patient's medical records if that individual provides the hospital with written permission and if applicant is 18 years or older. As a 'patient proxy,' you will have access to the individual's personal health information. Authorization for proxy access to patient's account is valid until revoked by the patient.

Test results are usually available in 72 hours. The portal contains information dating back to 2008. Medical records are as current as the last time the person was registered as an outpatient or inpatient at Doylestown Hospital. Records do not provide doctors' office reports. Patients can also print copies of their records from the portal.

The Doylestown Health Physician Practices and Urgent Care patient portal is a secure online website where patients can view health information at any time of day from anywhere with an Internet connection. You can access the site with a secure username and password.

If you have questions about your practice patient portal or are having trouble accessing your account, please call your doctor's office for assistance. Please note that medical records in the practice patient portal are separate from medical records in a hospital patient portal.

The Stamford Health and Stamford Health Medical Group Patient Portals help engage patients and families in their care by providing a secure and confidential web-based tool for accessing your health information. Please note you may have multiple portals depending on where you were treated and their features may vary. Learn more about each patient portal's offerings and how to enroll below.

As of April 5, 2021, Stamford Health will enhance the Stamford Health Medical Group and Hospital patient portals to comply with the recent 21st Century Cures Act. The Cures Act ensures patients have access to their electronic medical records via the patient portal without delay. All completed provider notes, laboratory tests and diagnostic tests will be available on the portal for both historical and recent visits, this may also include previously unavailable sensitive results such as HIV, STD, genetic testing, etc. Please make sure only appropriate individuals have access to your portal through the proxy access. If you have any questions about accessing the patient portal, please contact the Health Information Management Department at 203.276.7034. 2ff7e9595c
